Comparative Study of Fasting Plasma Glucose and Oral Glucose Challenge Test as Screening Method in Gestational Diabetes Mellitus

Abstract

This Prospective study was conducted over a period of 1 year on 100 pregnant. The aim was to compare fasting plasma glucose and oral glucose challenge test as screening method in gestational diabetes mellitus”. To screen and diagnose GDM early thus enabling early intervention. Study prevalence of GDM in the population tested. Women included in the study were subjected to fasting and oral glucose challenge test between 24 to 28 wks. Women with pre-existing diabetes, Women taking drugs known to cause hyperglycemia (corticosteroids, beta adrenergic agonists, thiazide diuretics and women with multiple pregnancies were excluded. Women with FPG >90mg/dl and OGCT with 50gms of glucose >130mg/dl were subjected to GTT with 100gms of glucose, two or more values above the threshold FPG - 95, 1hr – 180, 2nd - hr. – 155, 3rd hr. – 140 mg/dl were used for a diagnosis of GDM. Results of both the screening tests were subjected to statistical analysis.

Results

Prevalence of GDM was 28%. The Sensitivity of OGCT was high i.e. 89% as compared to sensitivity of FPG 64%. The specificity and positive predictive value of FPG was 100% and that of OGCT 97% and 92% respectively. Negative predictive value of OGCT was 95% that of FPG 87%. The sensitivity comparison is significant with P -value - 0.0267.

Conclusion

In the present study the 50-g OGCT with 130mg/dl as threshold level is considered to be an ideal test for the screening of gestational diabetes because of its high sensitivity as compared to fasting plasma glucose.
